I see this mistake quite often in that 饂飩屋 refers to the "physical" shop only. Better hang a -san on the end or you will be considered beyond rude; worse yet, not even understood.

As I am normally most polite and deferential, I shall continue in that vein. What brain-dead, dust-covered "expert" on the English language actually said that
患者様 means "patient" only. 様 is simply a polite form for "san", the mis-affixing of which will surely evoke much laughter and be rightfully perceived as 馬鹿丁寧; even funnier if one refers to himself in this way. The expression only means "patient", usually medical. As such it is only a "polite" form of 患者.

OK, here we go again, this entry does not mean "revenue" but rather "revenue office/bureau", the more commonly used being either 税務署 (most common) 税務局 (maybe). Again, I know this because a friend of mine whose husband worked for the 税務署 advised me against forming a 株式会社, because then I would have to pay taxes! I did and did. Oh my, dictionaries are still bad, aren't they? And I shall still be employable!
