Editorial Comment WRT 手, I'm not convinced that a change is needed. Koujien has three entries: て, しゅ and た. I think しゅ is plain onyomi usage. For た, Koujien says: 「て」の古形. I can't find a solo usage of 手/た. Maybe a note like "occ. pronounced た when a prefix".

For 上/うわ, there are solo cases, e.g. 上の空/うわのそら.
